Qualifications & Inclusions

Family Promise Program - Pre-Qualifying Criteria

  1. General

    • 3 year term to utilize all Family Promise services
    • PGT-A all embryos
    • Transfer all viable cryopreserved embryos before an additional egg retrieval
    • Willingness to undergo psychosocial education and counseling by a qualified mental health professional and engage independent legal counsel for each side of the egg donation and surrogacy arrangements
    • Meet Iliinois medical need for surrogacy as determined by our physicians
  2. Sperm Provider: Good sample as defined by WHO (World Health Organization) published criteria.

  3. Egg Provider: Your Egg Donor must be from the Chicago IVF network. Please note that all Chicago IVF fresh egg donors typically live locally to one of our locations in the midwestern USA.

  4. Gestational Carrier: Demonstrated ability to carry a pregnancy safely to term.

  5. CIVF Egg Donor: The program is only available when using an egg donor from the Chicago IVF network. Please note that all Chicago IVF fresh egg donors live locally to one of our 5 locations in northeastern Illinois and northwest Indiana.

  6. Three Years: The guarantee program remains valid for three years following the first egg retrieval.

  7. Genetic Tested Embryos: Before considering another egg donor retrieval, all viable embryos or PGT-A normal embryos must be utilized.

  8. XX or XY?: The program does not guarantee the sex (boy or girl).

  9. Non-Preferred Gender: If PGT-A normal embryos are of a non-preferred sex, an additional agreement may need to be signed. Additional costs are likely. Please clarify this point and ask that cost estimates are included in your financial estimate.

  10. Non-Refundable: Both the package fee and the program deposit are non-refundable. Choose this option only if you are comfortable with the possibility of paying more if your first IVF attempt is successful. This is, in fact, how the program is feasible: those patients who are successful on the first attempt basically fund those who require multiple attempts.

Medical IVF Clinics

The Family Promise Surrogacy Guarantee Program is exclusively available at Chicago IVF Clinics, a network with highly stable IVF success rates.

Surrogate Matching

For this program, if you have not independently matched with a friend or family member as your surrogate, you will need to choose an agency. The agency will provide the most accurate estimate of your non-IVF clinic expenses. Significant additional expenses ($100k est for two surrogates) will have to be paid by you. Please speak with your Family Promise program coordinator for additional information on choosing an agency and an estimated breakdown of typical non-IVF clinic costs.

Legal Requirements for IP’s and Surrogate

While the Family Promise Program doesn't include any non-medical fees, such as those for contract drafting, representation and parentage support, we understand the importance of having expert guidance throughout each stage of your surrogacy journey. To help you control costs and ensure your legal rights are protected, we can provide you with a list of qualified attorneys specializing in egg donation and surrogacy law.

Health Insurance for Surrogate and Baby

Health insurance is mandatory for both the surrogate and the baby. Key points to note:

  1. In some cases, the surrogate may already have a surrogacy friendly health insurance policy that can cover a portion of the costs.

  2. However, in other cases, the IP’s may need to purchase an additional insurance policy privately or through the healthcare marketplace or pay the full cost.

  3. Insurance premiums can vary significantly, ranging from several hundred dollars (a month) to tens of thousands of dollars per year.

  4. We collaborates with a group of insurance specialists who have a wide experience in surrogacy insurance. They can help you understand your options and find the most suitable coverage as per your needs.

  5. Our team is available for your guidance. You can discuss insurance with your Surrogacy Coordinator.

Services Included in the Program

The Family Promise Surrogacy Guarantee Program does not guarantee a live birth, but offers 3 years of unlimited attempts with controlled costs. This program is designed in a way that will minimize surprise expenses, although strict rules and criteria apply. It is very important that you are aware of this before making a commitment to this program.

Services included for Surrogate IVF Cycle

  • In-cycle ultrasound examinations & blood tests
  • Embryo creation, including fertilization, ICSI, AZH
  • PGT-A (standard 4-embryo package)
  • One-year embryo cryopreservation storage
  • Embryo Transfer Procedures

Services included for Surrogate

  • Testing & screening prior to treatment
  • Partner screening
  • Required HSG and/or sonoHSG
  • Pregnancy care (up to 11 weeks)

Program Exclusions

Some services are excluded, and these costs can be on the higher side. It is very important to work with a Financial Counselor to fully understand both included and excluded expenses.

Surrogate compensation is not included in the price of the program, and will be arranged through the surrogacy agency you choose.

Additional pre-cycle tests require additional fees, such as:

  • Non-standard procedures or costs associated with any complications
  • PGT-M single-gene Services
  • Surgical retrieval of sperm
  • Egg retrieval medications

Any service not performed at CIVF requires additional fees, such as:

  • Egg donor compensation and anesthesia after 2nd retrieval
  • PGT-A biopsy fee after 1st egg retrieval
  • GC surgeries (any type)
  • Embryo transfer medications
  • Any other service not performed at CIVF
  • Additional IP psychological evaluations (excluding the initial evaluation included in the program)
  • Legal fees for GC and/or Intended Parents
  • Embryo/Sperm cryopreservation storage after 1 year
  • PGT-A testing fee after 8th embryo
  • Cost of donor sperm
  • Additional GC screenings (excluding the initial evaluation included in the program)
